  By using this new time-saving Excel template, you can accurately provide us your group census by email or expedite the fulfillment of your quote request when you are using our QuoteStar™ online quoting system.
  
  Please save this template in your local drive. You can now input your group census data in Excel. For best results, please do not modify the format of the template.

  Upload to QuoteStar
  Once you have completed entering your group census data on the Online Census Template Form, you can directly upload this document to QuoteStar:

  • Log on the LISI website and go to “Online Quotes from QuoteStar.”
  • Click “Quotestar Group Quotes System” and log on to QuoteStar.
  • From the Main Menu page, create a new proposal or edit an existing one.
  • In the Group Data page, under the “Step 2–Census Detail” section, click the “Edit Census” button.
  • In the Census window, click the “Browse…” button to locate your saved Excel document on your local drive.
  • Click the “Update/Upload” button to upload the template. The census information will automatically populate the appropriate fields in the Census page.

  Attach in Email
  When submitting a request to your local Quoting department via email, you can attach the completed Online Census Template Form in the email, along with detailed information about your quote request (such as carriers, plan names, etc.). You are still welcome to submit your quote request by email or fax in the same manner as you have in the past, but you may find that this convenient tool will save you time.
